As far as hickeys go, they are a very obvious and outward sign that you're engaging in sexual activities. So rather than people reacting to a few broken blood cells, they are reacting to the idea that you are engaging in (as least) some serious making out - and then having it out there for the whole world to see.
I tend to think that many adults that are older than the people they are caring for - teachers, parents, etc - will get a bit judgmental when it comes to a younger person they feel responsibility for engaging in sexual things.
