Just as a doctor wouldn't be able to say for certain whether or not you were a virgin, nor would a guy.
However, sex with a female who is a virgin can be quite noticeable - her reaction to what can be anything from discomfort to pain usually gives the game away.
Not having sex for awhile will not change the makeup of your vagina. It is true, however, that regular sexual intercourse will make things easier for the woman - though this is only true at the very start of her sex life.
Therefore, sex might be uncomfortable for the first two or three times.
Beyond that, and the vagina should be able to cope without pain. This is because no matter how long you go without sex, you've already had it.
